Klinische betekenis:
De directe Coombs test of directe antiglobuline test wordt uitgevoerd voor het aantonen van de aan- of afwezigheid van antistoffen op erytrocyten waardoor deze gesensibiliseerd worden.
In-vivo binding van antistoffen op de erytrocyt komt voor bij:
• (Auto-immuun) hemolytische anemie
• hemolytische ziekte van de pasgeborene (icterus neonatorum)
• bij onderzoek naar (uitgestelde) transfusiereacties.
Ingangsdatum:
Bij de directe antiglobuline test worden de te onderzoeken erytrocyten geïncubeerd met antistoffen gericht tegen humane globulinen (antihumaanglobuline): anti-IgG, complementfactoren en anti C3d. Indien er IgG-antistoffen en/of complementfactoren in-vivo op de erytrocyt gebonden zijn, zal er agglutinatie optreden.
Matrix:
Via een goede venapunctie (zie SOP Bloedafname MA001/BL) wordt het bloed opgevangen in een vacuümbuis met EDTA. Van het EDTA bloed wordt een celsuspensie gemaakt zoals beschreven in punt 9.5.
